Where the US stands

Where the US stands: part of the ICD-11 Foundation course.

ICD-11

Three decisions

In the United States, three separate decisions are involved, according to the National Committee on Vital and Health Statistics: cause-of-death reporting, morbidity reporting for health care and public health, and payment.

Payment is the big one

Payment is the big one. ICD is a HIPAA code set, so changing diagnosis coding on claims needs federal rulemaking by HHS. The last move took about 25 years from WHO's endorsement of ICD-10 to its use for US claims.

Today's status, dated

As of 13 October 2026, we found no published US implementation date and no proposed rule. US claims stay on ICD-10-CM.

Procedures

And one more thing for the US: inpatient procedures are reported in ICD-10-PCS, which a move to ICD-11 for diagnoses would not by itself replace.
